The track features center-based cardiology rounding, 24/7 cardiac nursing oversight, in-house EKGs and echos, weekly heart failure rounds, and a structured recovery journey from stabilization through discharge and transition home. Cardiac rehospitalization rates across Vierra Communities run well below state and national benchmarks, a result of keeping cardiologists actively engaged throughout every stage of a patient's stay rather than handing off care at the hospital door.
