IntellaTriage, the nation's leading provider of nurse-first hospice and home health triage solutions, has launched an innovative data integration with CommonSpirit Health at Home, one of the nation's largest nonprofit healthcare systems.
BRENTWOOD, Tenn. - Marylandian -- IntellaTriage, the nation's leading provider of nurse-first hospice and home health triage solutions, and CommonSpirit Health at Home (CSHaH), one of the nation's largest nonprofit healthcare systems, went live with a secure integration between Homecare Homebase and IntellaTriage's triage platform, IntellaHub. Partners since 2022, the companies recognized that a deeper integration of their systems could greatly enhance their shared mission of improving patient and caregiver experiences. Home health and hospice electronic medical records (EMR) often lack interoperability, hindering efficient information exchange and patient care in the field. By breaking down these silos, both companies anticipate improved workflows, eliminated redundancies, and enhanced experiences for patients and field nurses.
Since launching its partnership with IntellaTriage, CommonSpirit Health at Home has witnessed significant improvements in patient outcomes and staff retention, attributed to the partnership and other strategic initiatives. The two organizations have continued to deepen their partnership, with the integration between the two organizations primary record keeping systems as a natural next step to future collaboration.
In 2023, IntellaTriage officially launched IntellaHub, a proprietary triage platform with features such as advanced analytics and benchmarking, improved communication between field and triage teams, and best-in-class triage algorithms. The integration with Homecare Homebase marks a major milestone for IntellaHub on the path to ensuring a completely seamless experience between daytime and after-hours care for patients and their families.
